YOUTH TRANSPORTATION FORM

    Please be sure to have all the necessary information when you are ready to schedule: 

• Rider’s name,
• Date of Birth, new customers only  
• Pick-up address,
• Appointment time and date,
• Drop-off location,
• Any additional stops
• Return time, if known
• Special mobility needs? (walker, wheelchair, etc.)

Messages and online ride requests are retrieved during business hours, so please keep in mind that weekend messages will be not received until Monday morning. The same applies to cancellation messages, so please try to let us know as far in advance as possible if you need to cancel a ride.

In many cases, you will share rides with others, which may mean an adjusted pickup time. We appreciate your patience and understanding.
